Healthcare startups are at a crossroads, debating whether AI should directly transform patient care for scale or primarily support clinicians behind the scenes. This fundamental divergence shapes the future of medicine, balancing efficiency with human connection.

The future of healthcare, long envisioned as a tapestry of compassionate care and scientific advancement, is increasingly being woven with threads of artificial intelligence.

But within the burgeoning world of healthcare startups, a fundamental divergence is emerging, creating two distinct pathways for how this powerful technology will reshape patient interactions, clinician workloads, and ultimately, the bottom line.

It’s a high-stakes gamble, pitting the promise of exponential efficiency against the inherent complexities of human health and empathy.

On one side of this evolving landscape are the audacious innovators, those who see AI not merely as a tool for administrative convenience, but as a direct conduit to scalable care.

For these companies, the vision is clear: leverage AI to dramatically multiply patient caseloads, streamline clinical processes, and unlock unprecedented profitability in an industry traditionally plagued by tight margins and clinician shortages.

Consider Hinge Health, the physical therapy giant, which boasts that AI has slashed 95% of clinician hours, empowering its therapists to manage a greater volume of patients.

Their co-founder and CEO, Daniel Perez, doesn’t mince words, declaring that “at some point, whether 10, 50, or 200 years in the future, care delivery will be automated with technology. And that’s a good thing.” This philosophy underpins their aggressive use of computer vision for at-home physical therapy sessions and AI-powered messaging, pushing the boundaries of what virtual care can achieve.

Sword Health, another major player in musculoskeletal care, mirrors this ambition.

They aimed for their physical therapists to manage an astounding 700 patients by the end of 2024, a significant leap from the 200-300 at the year’s start.

Their strategy involves AI pre-writing patient messages and prioritizing those in greater need, effectively turning AI into a force multiplier for human expertise.

Similarly, Summer Health, a virtual pediatric care startup, envisions AI doubling the number of patients its text-messaging-based providers can see in an hour, from 10 to potentially 20.

Founder Ellen DaSilva sees this not as a replacement for doctors, but as a critical augmentation, especially in specialties facing severe clinician shortages.

The underlying belief here is that technology can “peel away aspects of in-person care,” delivering quality through intelligent automation.

Virta Health, a chronic care company focusing on diabetes, offers a compelling financial testament to this aggressive integration.

Since 2017, they’ve embedded machine learning into their operations, and with the advent of generative AI, their gross margins have surged from a negative figure to an impressive 60%.

CEO Sami Inkinen highlights AI-powered personalized care plans and 24/7 chatbots for nutrition queries, though he emphasizes that AI isn’t making clinical decisions or replacing providers outright.

Their focus remains on outcomes, with financial improvements following suit.

These companies are, in essence, leaning into the promise of AI as a direct conduit to efficiency and scale, believing that a well-designed algorithm can extend the reach of human clinicians far beyond traditional limits.

Yet, a palpable sense of caution defines the other side of this healthcare AI coin.

For a significant cohort of startups, the human element remains sacrosanct, particularly when dealing with sensitive patient data and complex health conditions.

These innovators are adopting AI, but keeping it firmly “at arm’s length” from direct patient interaction, channeling its power into administrative efficiencies that support clinicians rather than supplant them.

Their concerns are rooted in patient safety, the nuanced experience of care, and the elusive quality of empathy – attributes that algorithms, for now, struggle to replicate.

Omada Health, a chronic care peer to Virta, exemplifies this more measured approach.

While they’ve long utilized AI behind the scenes for tasks like suggesting educational videos, they’ve been notably slower to introduce patient-facing AI tools.

Their first such offering, an AI agent for nutrition questions, only debuted recently, a move made only after significant improvements in large language models and extensive training on their own clinical data.

Omada co-founder and CEO Sean Duffy articulates the core of their philosophy: “I’ve yet to find someone who feels accountable to Chat GPT.” He pointedly notes the existence of “artificial intelligence” but the absence of “artificial empathy,” underscoring their commitment to a “proactive people component” in care delivery.

For Omada, AI is an enabler, not a replacement for genuine human connection.

Similarly, NOCD, a mental health startup specializing in obsessive-compulsive disorder, is deploying AI to tackle the often-burdensome administrative load.

Clinical note dictation and revenue cycle management are now handled by AI, contributing to the company’s newfound profitability and allowing them to expand their therapist network without ballooning operating expenses.

While they have a chatbot, Robin, it serves as a resource for therapists during sessions, never directly interacting with patients unless explicitly prompted by the clinician.

CEO Stephen Smith’s caution is particularly pronounced, given the severity of the mental illness they treat.

“All these different projects, we know we have to proceed with some caution since we’re dealing with a severe group of people,” Smith states, emphasizing the need for extensive research to ensure effectiveness and a “safe experience” for both members and therapists before considering AI for direct clinical care.

This dichotomy highlights a critical juncture for the healthcare industry.

Can the relentless pursuit of efficiency, enabled by AI, coexist harmoniously with the deeply personal and often vulnerable nature of patient care?

The “all-in” camp believes automation is not just desirable but inevitable and beneficial, a necessary evolution to overcome systemic challenges like clinician burnout and access to care.

The “arm’s length” proponents, however, argue that true care requires accountability, empathy, and human judgment that AI, in its current form, cannot replicate.

They see AI as a powerful backend engine, freeing up clinicians to focus on what only humans can provide: genuine connection and nuanced clinical decision-making.

The paths these startups choose will undoubtedly shape the future of medicine.

Whether AI becomes a pervasive, patient-facing caregiver or remains a sophisticated administrative assistant will determine not only the profitability of these ventures but also the very quality and character of healthcare for generations to come.

The debate isn’t merely technological; it’s profoundly ethical, challenging our definitions of care, connection, and what it truly means to heal.
